Grantee Reports
Recipients of grants from the Circle of Service Foundation must provide an annual grant evaluation report showing how the grant funds were used and an internal evaluation of the program. The Foundation will not accept new grant requests until all required reports for prior grants have been filed.
Please see the following links for recommended forms for reports. You are not required to use these forms, but, if you submit a report in a different form, please ensure that it includes at least as much information as provided for in the applicable reccomended form.
- Final Report Form: Please use this form to provide your report at the conclusion of a grant.
- Annual Progress Report Form: Please use this form to provide your report on the anniversary of a multi-year grant. Only a final report is required for one-year grants.
- Challenge Report Spreadsheet: A Challenge Report must include (a) this spreadsheet detailing the matched donations or in a form of your own (as long as your form contains no less information than that requested by our form) and (b) a letter signed by both your executive director and an officer of your board of directors, attesting to the submitted spreadsheet, stating that you have met the challenge to the extent of the requested payment and stating that you have complied with the terms and conditions of this grant.
Budget Templates
As indicated in our application, we have recommended templates for agency budgets and, if applicable, project budgets. You are not required to use these templates, but, if you submit a budget in different form, please ensure that it includes at least as much information as provided for in the applicable template.
