Grantee Reports
Recipients of grants from the Circle of Service Foundation must provide an annual grant evaluation report showing how the grant funds were used and an internal evaluation of the program. The Foundation will not accept new grant requests until all required reports for prior grants have been filed.
Please see the following links for recommended forms for reports. You are not required to use these forms, but, if you submit a report in a different form, please ensure that it includes at least as much information as provided for in the applicable recommended form.
Programming Reports
- Final Report Form: Please use this form to provide a programmatic report at the conclusion of a grant. This is required at the conclusion of all grants.
- Annual Progress Report Form: Please use this form to provide your programmatic report on each anniversary of a multi-year grant. This is required annually for all multi-year grants.
Challenge Reports
- Challenge Report Form: Please use this form to request a payment. You may want to refer to our Tracking and Reporting FAQs where we describe the process for requesting payments.
- Challenge Update Form: Please submit a Challenge Update by the Challenge Update due date listed in your grant terms. If you submit a Challenge Report before or on the date the Challenge Update is due, you do not need to submit a Challenge Update.
- Challenge Report Spreadsheet template:You are welcome to use this template for Section A of the Challenge Report. You are not required to use our template, but if you use a different form, please ensure that it includes all of the information requested in Section A.
